OCSACC Forms
The OCSACC programs are licensed by the Virginia Department of Social Services. We are, therefore, required to collect and maintain certain information on each child enrolled. Please find our registration packet, parent handbook, and various other OCSACC related forms below.

Medication Administration

Please refer to our Parent Handbook for our complete policy on medication administration. Included below is a quick description of which forms need to be completed in the event your child needs to receive medication while attending one of our programs followed by our medication administration authorization forms.

Short Term Medication (ten days or less)

  • Over the counter products – administration method: topical – includes sunscreen, oral, Inhaled/nasal, patches, eye, and ear will require WRITTEN Medication Authorization form signed by PARENT
  • Prescription – administration method: topical, oral, inhaled/nasal, patches, eye and ear will require WRITTEN Medication Authorization form signed by PARENT
  • Prescription – NEBULIZER & EPI-PEN injections will require WRITTEN Medication Authorization form signed by PHYSICIAN

Long Term Medication (more than 10 days)

  • Over the counter products – administration method: topical – includes sunscreen will require WRITTEN Medication Authorization form signed by PARENT
  • Over the counter products – administration method: oral, inhaled/nasal, patches, eye and ear will require WRITTEN Medication Authorization form signed by PHYSICIAN
  • Prescription – administration method: topical, oral, inhaled/nasal, patches, eye and ear, Nebulizers and Epi-pen Injections will require WRITTEN Medication form signed by PHYSICIAN

Medication Authorization Form - Long/Short Term
*Use this form for medications including, but not limited to, inhalers, ADD/ADHD medications, Tylenol, Motrin, allergy medication.

Medication Authorization Form - Over-the-Counter Skin Products
*Use this form for medications including, but not limited to, inhalers, ADD/ADHD medications, Tylenol, Motrin, allergy medication.
